Common Simplex 4005 Trouble Codes
- ZONE OPEN: A field wire on an initiating device circuit has been disconnected or severed, preventing the panel from reading the end of line resistor.
- ZONE SHORT: A positive and negative wire are touching on an initiating circuit, which can trigger a false alarm or an isolated trouble condition.
- GROUND FAULT: A bare copper wire on a zone or notification circuit is physically touching the metal conduit or the panel motherboard enclosure.
- BATTERY TROUBLE: The sealed lead acid backup batteries have dropped below their required voltage or failed an automatic load test.

The Simplex 4005 is a severely outdated conventional fire panel, making replacement parts incredibly difficult to source.
